Okular (in KDE 4.1) allows embedding of notes, comments, reviews and 
underlining in PDFs. This is not exactly a PDF note that is carried with 
the PDF everywhere; but it's available to you and it's saved 
automatically.

If you would like to export the document WITH annotations, you can save 
it as "Document Archive"; then you can open it with all the annotations 
in any copy of Okular, everywhere.
