Coming from a QA background, it's entirely possible (and if this were 
the cheap@$$ company I used to work for it's more likely) that QA did 
their job's correctly & reported the problem whereupon the company 
looked how how much it would cost to pay the programmers to correct the 
code and decided it was good enough as is.
